Small Custom Order Form

This order form is to go through the basics of getting your very own BeRazzled piece of Art! There are several questions for you to answer, but the more detailed your request, the more it fuels my creativity to bring what you're looking to get to life! Sometimes I may reach out to a customer to ask if a certain design suits their fancy better and to try and keep you involved with the making of your item. So please make sure you have accurate contact information. I will leave a box at the bottom for your additional requests and ideas.

PICK A STONE! ANY STONE! Once you choose your stone type, I will get in touch with you to display my available stones for you to pick. Here you can choose from my Smooth Tumbled, Raw or Cabochon collection:

I CANNOT stress how important it is for me to have your measurements. If I receive no measurements(Preferable in INCHES or FEET), I will cancel the order within 5 days. I am not responsible for any inabilities to fit said piece due to inaccurate measurements. If you want your piece to be perfect for you, take the time out to measure yourself or the person you're purchasing for. Otherwise there will be an additional charge for alterations. I wish I could be everywhere and just pull up on you with a measuring tape! Lol! That'd be cool. I've provided a small space below for you to add your measurements and details.

Please allow me 2-4 weeks to create your piece. More than likely, I'll be in contact while working on the piece for clarity sakes. Upon completion of your piece, I always send approval pictures to ensure you're satisfied with your product before ever sending them out.

****The Deposit amount is due IMMEDIATELY and is NON-REFUNDABLE**** It will go towards the total price of your piece. You have 48 hours to reach out to cancel the request from the time of submission. The only exception is if I am unable to create the piece for you. I will promptly return your deposit to you. Once your item has been made, you have 45 days to pay the remaining balance. Otherwise, the deposit will be forfeited and the piece will be set out for sale at full price. ***
